We suggest you arrive 15 minutes before your scheduled appointment to allow our staff to process your paperwork in a timely manner.

How to schedule your family medicine appointments

To better serve our patients and to minimize your wait time, please make separate appointments for each family member to be seen by the doctor.
Our Healthcare team wants to address all of your medical concerns. In order to adequately address all concerns we must schedule the appropriate amount of time. When scheduling an appointment please:

  • Share with the scheduler the reason for your visit.
  • Schedule separate appointments for multiple concerns.

What to bring to your appointment

To insure that insurance claims are being processed correctly and that we are up to date on any changes in your treatment plans being administered by another provider, please bring the following items to each office visit:

  • Current insurance card(s)
  • Co-payment(s)
  • Photo ID
  • All current medications and dosing instructions (actual prescription bottles are preferred)
  • Over the counter supplements or vitamins (actual bottles are preferred)

We also request you bring any information pertinent to your office visit:

  • Home blood pressure readings
  • Bood sugar readings.

If you need to cancel your appointment

Your time is valuable to us. Patients are seen by appointment only. If you must cancel your appointment, please give us 24 hours notice, providing an opportunity for another patient to be seen. We’ll be happy to reschedule your appointment at a more convenient time.

If you need a prescription refill

We prefer that prescription refill requests are discussed with the physician at the time of your appointment. In the event that you must request a refill in between your routine visits, please allow 24 hours notice. Many health conditions require careful monitoring, and we generally cannot fill prescription requests unless you have been seen by the physician or nurse practitioner in the last three months.

If you are a family member or caregiver

We encourage family involvement in the care of dependent family members. Please make sure we have copies of any Living Will or Durable Power of Attorney on file. Please make sure a HIPAA release form is on file listing all persons with whom we can discuss information regarding the patient.
We are careful to protect the privacy of our patients.
